HomeCare+ services
Household chores:
Your care worker can help with common chores such as trash removal, meal planning and preparation, laundry, housekeeping tasks, and more.
Personal care assistance:
HomeCare+ providers can also help with bathing, oral hygiene, dressing, skin care, medication reminder, and transferring and mobility assistance.
Respite care:
Provide relief and/or assistance for the caregiver in the comfort of your own home, including overnight stays as needed.
Home HealthCare+ services
Home HealthCare+ includes everything from HomeCare+ while also offering:
Skilled nursing:
Visits are for specific hands-on skilled medical care services for you and information gathering for your physician. We train you and your family members on new care and discuss individualized goals based on your current medical needs. Your goals are our goals.
Home Health Aide:
Visits allow you to manage your daily living activities in your home. Services may include bathing assistance, dressing, mobility, light housekeeping, and meal prep.
Private nurse duty:
Services help you in the home in the same way as visits but for longer periods of time. A nurse will assist you if you are no longer able to safely remain in your home without support when a family member is at work or out of the home.
How to get started
1. Evaluation
Once you contact us, our Care+ staff conducts a preliminary evaluation of your potential eligibility.
2. In-person meeting
A face-to-face meeting is held, where we gather additional information as well as listen to your goals.
3. Service Plan
If you’re eligible, our dedicated staff will create a Consumer Service Plan with a selected provider.
4. Services Begin
Once the plan is received, the Care+ staff assigns a worker, creates a schedule, and sets a start date for the services.
